In most cases, front sleepers tend to turn their head to one side. This movement puts stress onto the neck because it creates tension in the muscles and ligaments, and so it causes distress and pain on waking up. In addition, this position can stimulate gastro-oesophageal reflux. Physical problems also extend to the vertebral column, giving rise to back pain on waking up. One of the recommendations for people who sleep in the prone position is to try it without a pillow under the head, in order not to stress the neck.
